Amosup Endorsements

AMOSUP RETIREMENT FUND WITHDRAWAL:

If you are a member of AMOSUP (Associated Marine Officers’ & Seamen’s Union of the Phils.), you are entitled to Provident Fund which is being paid by your employer.

WHO CAN AVAIL?

  • ·         Qualified member
  • ·         Legal dependents (in case of disability or death)

QUALIFICATIONS:

  • ·         Any of the below eligibility:
  • ·         Retirement age of 50
  • ·         2 years inactive from a certain agreement
  • ·         10 years sea service on a participating agreement
  • ·         Disablement
  • ·         Death

DOCUMENTS REQUIRED:

  • ·         ID, passport or Seaman’s Book
  • ·         Medical certificate (to claim for disabled member)
  • ·         Provident fund passbook
  • ·         Application for payment of retirement
  • ·         Personal letter of request, stating reason for claiming benefit, addressed to the President of AMOSUP –                   Dr. Conrado Oca.
  • ·         Endorsement letter from agency

Please secure your provident fund passbook and endorsement letter from Ms. Mildred Tadeo of the Crew & Family Welfare Section of Aboitiz Jebsen.  For this to be processed  in advance,  you can either  submit a request via  our  online  REQUEST  CERTIFICATES link  or send  an email  to css.group@abojeb.com.ph.

 

Once requirements are completed, this shall be submitted at AMOSUP office with a maximum of 1 month processing.

 

AMOSUP SAILOR’S HOME AVAILMENT PROCEDURES:

The Sailor’s Home was conceived to address the board and lodging needs of transient members who reside outside the National Capital Region.

Reservation Procedures & Requirements (Manila only):

 

  • ·         AMOSUP ID and/or Seaman’s Book are required for reservations.  A Check-in/Reservation Form is issued which you need to accomplish.  Then, go to the Sailor's Home Administrator at the Seamen's Center for approval.
  • ·         After presenting the approved form, you will be issued with a reservation slip indicating the date of reservation.
  • ·         Reservations are also entertained through phone calls.  When the Sailor's Home is fully booked, you will get scheduled on first-come-first-served basis.
  • ·         In case reservations will not push through due to  no show up on the designated date, the you need to advise the Sailor's Home office (tel. 527 3605).

 

*Use of the Sailor's Home facilities is free-of-charge including breakfast.

 

AMOSUP HOSPITALIZATION AVAILMENT:

The Seamen’s Hospital is one of the operating arms of AMOSUP which caters to the medical needs of all AMOSUP members and their qualified dependents. Information on Qualification, Documentary Requirements for Availment of the AMOSUP FMDP Program Benefit, Hospital Services by Service Unit, and Available Procedures By Service Unit.

WHO CAN AVAIL?

  • ·         All active bonafide members of the AMOSUP

Qualified Dependents

For Single Members

  • ·         Natural parents
  • ·         Unmarried brothers and sisters below 18 years old

For Married Members

  • ·         Legitimate Wife and
  • ·         Unmarried children below 18 years old

REQUIREMENTS:

For Seaman:

  • ·         AMOSUP Member’s ID & Seaman’s Book

In the absence of the above, a verification slip of member’s 201 file may be used temporarily.

For  DEPENDENTS:

  • ·         Allotment slip of the current vessel with active CBA with AMOSUP or allotment slip of previous vessel with active CBA with AMOSUP provided that the member’s date of disembarkation is within 6 months.

In the absence of the above, copy of the ff: may be used temporarily:

  • ·         Contract of crew
  • ·         Latest Pay slip
  • ·         Certification from the Manning Agency bearing the name of the current vessel with  active CBA with AMOSUP.
  • ·         Verification slip of  member’s 201 file .
  • ·         AMOSUP Dependent’s ID
     (WIFE-red background / PARENT-yellow /CHILD-green)

In the absence of the above, copy of the ff: may be used temporarily:

  • ·         Birth certificate w/ registry number (NSO copy)-child
  • ·         Marriage certificate w/ registry number(NSO copy)-wife

HOSPITAL SERVICES

  • ·         Only available services and medicines can be availed for free at AMOSUP Seamen’s Hospitals.
  • ·         Prescription and request for diagnostic examinations i.e. chest x-ray, ct scan, laboratory tests, etc) from an outside physician* is not accepted. (*doctors who are not connected with AMOSUP Seamen’s Hospital).
  • ·         Medicines are dispensed only in the presence of the patient (OPD cases). The AMOSUP Seamen’s Hospital strictly imposes the “NO PATIENT, NO MEDICINE” policy.


AMOSUP SLOP-CHEST AVAILMENT (MANILA & CEBU)

This arm of the union enables members and/or their allottee to purchase basic household needs and other items on credit basis when their spouses who are union members are on board. The Slop Chest or mini-mart allows the purchase of competitively priced basic commodities including appliances, on an interest-free, deferred-payment basis.

NORWEGIAN INTERNATIONAL SHIP (NIS) BENEFITS:

RETIREMENT /DISABILITY AND DEATH BENEFIT:

The Norwegian International Ship Register (NIS) Provident Fund is extended to Filipino seafarers serving on board vessels registered with the Norwegian International Ship Register.

QUALIFICATIONS:

  • ·         Retirement age of 50
  • ·         Disablement
  • ·         Death

DOCUMENTS REQUIRED:

  • ·         A completed claims form
  • ·         Copy of personal identification papers (seaman’s documentation)
  • ·         Copy of membership card (if possible)
  • ·         Death Certificate –( for deceased member only)
  • ·         Certificate of disability approved by Norwegian authorities – (for disabled member)
