The NSA Provident Fund is established by the CBA between the Norwegian Shipowners' Association and AMOSUP for the benefit of the Filipino seafarers serving onboard vessels registered with NIS.
The Provident Fund provides a lump sum cash payment at retirement age (50), disablement as a result of injury or disease while in service or death.
Contributions are paid into the Fund on behalf of the crew during the time he is onboard a participating vessel.

1. Please secure and complete a Uni Storebrand Claim Form from the Customer Support Services staff (Please look for Dara or Roge.)
2. Please prepare the following attachments:
- Letter addressed to Uni Storebrand
- Original copy of USL Membership Card
- Original copy of Service Record
- Photocopy of Norwegian Seaman's Book
- Certified True Copy of Birth Certificate***
- Original copy of USL Statement of Account
- Bank account number, bank name and complete address of bank
- Certified True Copy of Death Certificate*
- Certified True Copy of Marriage Contract*
- Original copy of Certificate of Disability from 2 USL accredited doctors**
* for Death benefit claim only
** for Disability benefit claim only
*** Retirement Age - 50 years old
Upon completion of the above-mentioned documents, kindly mail all documents to:
