PhilHealth
A seafarer MUST apply with Philhealth for Voluntary Payment Program whenever ON VACATION to prevent any lapse in contribution that may result to denial of claims once confined in a hospital.
Procedure: 1. Go to the nearest Philhealth office. 2. Apply for Voluntary Payment Program. 3. Pay a minimum fee of PhP300.00 covering a quarter period (3 months). Keep your Official Receipt for reference whenever necessary. Continue to pay after every quarter for as long as you stay on vacation. 4. Use only one and same Philhealth ID No. (PIN) as declared in Aboitiz Jebsens’ records. If unknown, contact Jebsens’ office c/o CSS Group. In the absence of Philhealth ID, use SSS No. as declared in Jebsens’ records and accomplish Philhealth Form M1A for application of PIN.
